c.        If insured items are installed in different aircraft, provided that, the Insured shall notify the Insurer immediately of this alterations.
d.       The Insurers shall not be liable for loss or damage which has been covered under an Aircraft Hull Insurance Policy”
22.    Mobile use outside of the Insured location (items in vehicle) for Interest Insured II.
“Notwithstanding of anything in the printed condition, it is agreed and understood that, the Insurer will indemnify the Insured for loss of, or damage to :
a.       Electronic Equipment if it was installed in, or on top of vehicle or trailer having a hardtop roof and is moved within the territorial boundaries as covered in the Policy
b.       The Insurers shall only be liable to pay for loss through theft or burglary if the vehicle was properly locked after having been parked.”
23.    Mobile use outside of the Insured location (items in water borne vessels) for Interest Insured II.
“Notwithstanding of anything in the printed condition, it is agreed and understood that, the Insurer will indemnify the Insured for loss of, or damage to :
a.       Electronic Equipment if it was installed in or a waterborne vessel.
b.       The Insured location shall be the waterborne vessel 
c.        If insured item are transferred over to another waterborne vessel, provided that, the Insured shall notify the Insurer immediately of this alterations.
d.       The Insurers shall be liable for loss or damage caused by mines, torpedos or similar.

c.     If the insured item was (burglary/theft) out of vehicle, the Insurer shall only be liable to pay indemnity if :

Special Conditions Concerning Fire-Fighting Facilities and Fire Safety on Construction Sites

Special Conditions Concerning Fire-Fighting Facilities and Fire Safety on Construction Sites

It is agreed and understood that otherwise subject to the terms, exclusions, provisions and conditions contained in the Policy or endorsed thereon, the Insurers shall only indemnify the Insured for loss or damage directly or indirectly caused by or resulting from fire or explosion, provided always that :


1.    With regard to the progress of work adequate fire-fighting equipment and sufficient extinguishing agents are available and operative at all times.


Fully operative wet riser hydrants are installed up to one level below the highest current work level and are sealed by temporary end caps.

2.    The cabinets containing hose reels and portable fire extinguishers are inspected at regular intervals but at least twice a week.

3.    Fire compartments as required by local regulations are installed as soon as possible after the removal of formwork.

Opening for lift shafts, service ducts and other voids are provisionally closed as soon as possible but not later than at the commencement of fit-out work.

4.    Waste material is removed regularly. All floors undergoing fit-out are cleared of combustible waste at the end of each working day;

5.    A "permit to work" system is implemented for all contractors engaged in "hot work" of any kind such as but not limited to :

-       grinding, cutting or welding operations,

-       use of blow lamps and torches,
-       application of hot bitumen,

or any other heat producing operation.


“Hot work” is carried out only in the presence of at least one worker equipped with a fire extinguisher and trained in fire-fighting.



The area of any “hot work” is examined one hour after the work has finished.

6.    Storage of material for the construction or erection shall be subdivided into storage units not exceeding the value stated below per storage unit. The individual storage units shall be either at least 50 m apart or separated by fire-proof walls.


All inflammable material and especially all inflammable liquids and gases shall be stored at a sufficiently large distance from the property under construction or erection and any hot work;


7.    A Site Safety Coordinator is appointed.


A reliable fire alarm system is installed and whenever possible a direct communication link maintained with the nearest fire brigade.



A fire Protection Plan and a Site Fire Action Plan are implemented and updated regularly.


The contractor's personnel are trained in fire-fighting and fire-fighting drills carried out weekly.

The nearest fire brigade is familliarized with the site and immediate access maintained for it at all times.

8.    The site is fenced off and access controlled.

Kondisi Khusus Mengenai Fasilitas Pemadam Kebakaran dan Keselamatan Kebakaran pada Lokasi Konstruksi

Dengan ini disetujui dan dipahami bahwa lain daripada yang diatur dalam syarat, pengecualian, ketentuan dan kondisi yang terdapat pada Polis atau yang diendos di dalamnya, Penanggung hanya memberi ganti rugi Tertanggung untuk kerugian atau kerusakan langsung atau tidak langsung yang disebabkan oleh atau diakibatkan oleh kebakaran atau ledakan, selalu dengan syarat bahwa :

1.    Berkaitan dengan kemajuan pekerjaan peralatan pemadam kebakaran yang memadai dan bahan pemusnah api yang cukup tersedia dan berfungsi pada setiap saat.

Hidran tipe wet-riser yang berfungsi penuh dipasang sampai dengan satu tingkat di bawah tingkat tertinggi yang sedang dikerjakan dan disegel dengan tutup sementara di ujungnya.


2.    Lemari penyimpan selang dan tabung pemadam kebakaran diinspeksi secara berkala sekurang-kurangnya dua kali seminggu.

3.    Ruang tahan api sebagaimana disyaratkan peraturan setempat dipasang segera setelah pemindahan form-work.

Bukaan untuk kolom lift, saluran pendukung dan bukaan lainnya sementara waktu ditutup sesegera mungkin tetapi tidak lewat dari dimulainya pekerjaan penyempurnaan.

4.    Sampah material dibuang secara berkala. Seluruh lantai yang sedang dalam   penyempurnaan dibersihkan dari sampah mudah terbakar pada setiap akhir hari kerja;

5.    Sistem “ijin kerja” diterapkan kepada semua kontraktor yang terlibat dalam semua jenis “pekerjaan panas” tetapi tidak terbatas pada:

-       pekerjaan penggerindaan, pemotongan atau pengelasan,
-       penggunaan lampu pijar dan obor,
-       pemakaian aspal panas,

atau pekerjaan lainnya yang menghasilkan panas.

“Pekerjaan panas” hanya dilakukan dengan kehadiran sekurang-kurangnya satu orang pekerja yang dilengkapi dengan satu alat pemadam kebakaran dan terlatih dalam pemadaman kebakaran.

Daerah “pekerjaan panas” diperiksa satu jam setelah pekerjaan selesai.

6.    Penyimpanan material untuk konstruksi atau pemasangan harus dibagi ke dalam unit-unit penyimpanan tidak melebihi nilai yang tercantum di bawah ini   tiap unit penyimpanannya. Tiap  unit penyimpanan harus terpisah dengan jarak sekurang-kurangnya 50 m atau dipisahkan oleh tembok tahan api.

Semua material yang mudah terbakar dan khususnya semua cairan dan gas yang mudah terbakar harus disimpan pada jarak yang cukup jauh dari harta benda yang sedang dalam pengerjaan konstruksi atau pemasangan dan setiap pekerjaan panas;

7.    Seorang Koordinator Keselamatan Lapangan ditunjuk.

Sistem alarm kebakaran yang dapat diandalkan dipasang dan bilamana memungkinkan sambungan komunikasi langsung dengan markas pemadam kebakaran terdekat dipelihara.

Program Perlindungan Kebakaran dan Program Tindakan Penanggulangan Kebakaran di Lokasi diterapkan dan diperbaharui secara berkala.

Pekerja kontraktor dilatih pemadaman kebakaran dan latihan pemadaman kebakaran dilaksanakan seminggu sekali.

Regu pemadam kebakaran terdekat yang terbiasa dengan lokasi dan jalan masuk terdekat dijaga setiap saat.

8.    Lokasi   dipagari dan jalan masuk dikontrol.

It is agreed and understood that otherwise subject to the terms, exclusions, provisions and conditions contained in the Policy or endorsed thereon, the Insurers shall only indemnify the Insured for loss, dam­age or liability arising out of earthquake if the Insured proves that the earthquake risk was taken into account in design according to the official building codes valid for the site and that the qualities of material and workmanship and the dimensions on which the calculations were based were adhered to.

Dengan ini disetujui dan dipahami bahwa lain daripada yang diatur dalam syarat, pengecualian, ketentuan dan kondisi yang terdapat pada Polis atau yang diendos di dalamnya, Penanggung hanya memberi ganti rugi Tertanggung untuk kerugian, kerusakan atau tanggung jawab yang timbul dari gempa bumi jika Tertanggung membuktikan bahwa risiko gempa bumi itu telah dimasukkan dalam perhitungan dalam rancangan sesuai dengan peraturan resmi atas bangunan yang berlaku untuk lokasi tersebut dan kualitas material dan pengerjaan dan dimensi atas mana perhitungan tersebut didasarkan ditaati.

SPECIAL CONDITION CONCERNING X-RAYS FILMS

It is agreed and understood that otherwise subject to the terms, exclusions, provisions and conditions contained in the Policy or endorsed thereon and subject to the Insured having paid the agreed extra premium, the Insurers shall not indemnify the Insured in respect of any costs incurred in connection with any loss of or damage to films used for X-ray equipment unless such loss or damage arises in direct consequence of an indemnifiable loss of or damage to the X-ray film cassettes.

SPECIAL CONDITION CONCERNING WAIVER OF SUBROGATION

It is agreed and understood that otherwise subject to the terms, exclusions, provisions and conditions contained in the Policy or endorsed thereon and subject to the Insured having paid the agreed extra premium, the Insurers shall waive all rights of subrogation arising out of loss or damage indemnifiable under the Policy which the insurers may have against persons using the insured items with the consent of the Insured.

SPECIAL CONDITION CONCERNING OBSOLETE EQUIPMENT

It is agreed and understood that otherwise subject to the terms, exclusions, provisions and conditions contained in the Policy or endorsed thereon, the Insurer’s liability in respect of electronic data processing equipment the manufacture of which has been discontinued, or for which spare parts are no longer available (obsolete equipment), shall be restricted to the period of business interruption which would arise if a current model or type of data processing system of equal performance had to be repaired or replaced.

Special Condition Concerning EDP Equipment Insured in connection Hire/Lease contract

It is agreed and understood that otherwise subject to the terms, exclusions, provisions and conditions contained in the Policy or endorsed thereon, the following shall apply in respect of EDP equipment insured under a separate policy in connection with a hire or lease contract:

Section 2 and/or 3 of the Policy shall be read as if the EDP equipment were insured under Section 1 of the Policy. This shall apply only up to the actual extent of cover provided by the separate policy, but in no case beyond the cover given under Section 1 of the Policy.

Cover of Valves and tubes

It is agreed and understood that otherwise subject to the terms, exclusions, provisions and conditions contained in the Policy or endorsed thereon and subject to the Insured having paid the agreed extra premium, this insurance shall be extended to include loss of or damage to valves and tubes. Indemnification shall be limited to the actual value of such items immediately prior to the occurrence of the loss or damage, including ordinary freight, erection costs and custom duties and dues, if any.

COVER OF MOBILE AND PORTABLE EQUIPMENT

It is agreed and understood that otherwise subject to the terms, exclusions, provisions and conditions contained in the Policy or endorsed thereon and subject to the Insured having paid the agreed extra premium, this insurance shall be extended to include loss of or damage to the mobile and/or portable equipment contained in the schedule of the Policy whilst stationary or in transit anywhere within the [……] territorial limits.

The Insurers shall not be liable under this Endorsement for
4 loss or damage occurring whilst the above items are unattended unless locked inside a building or vehicle
4 loss or damage from any cause whatsoever whilst the above items are installed or carried in or on aircraft or aerial devices or waterborne vessels or craft
